As I see from your Software log, IMEI was damaged in your device before the unlocking process. Upload somewhere QCN and Security backup files ("D855_Bad_13-10-2014_14-37-45.qcn", "D855_Bad_13-10-2014_14-49-19.SEC") and post here links, we will research, where can be the problem with your device.

Also, You have to perform the Factory data reset to complete the operation:
