What is exact state of the phone? Why did You decided to re-flash it?

i have same problem ,. phone have D820 firmware and inside phone model d821
i want to flash i have same error i do many times unlock bootloader after restart bootloader locked again

Try to reflash the phone with "nexus 5 root software" from the Support Area. If problem will remain the same - most likely device has HW problem.
