Hello.
Please re-flash the phone with "LGE960AT-00-V10e-NXS-JUL-29-2013-JWR66V-USER+0.oct" firmware from Support Area and tell results.

"LGE960AT-00-V10e-NXS-JUL-29-2013-JWR66V-USER+0.oct" firmware is dedicated specially for 16Gb version of the phone. Most likely this problem is not caused by firmware.
Have You tried to make Factory Reset?

The problem is with your device firmware. Write all firmwares from the Support Area one by one. Also, we will try to find and upload new other firmwares.

Try to make Factory Data Reset. Try to re-flash the phone with different firmwares from the Support Area.

ONLY THIS IS THE SOLUTION WORKS FOR ME:

YOU DONT NEED FLASH THE FIRMWARE OF THIS GUIDE, YOU CAN FLASH ANY OF OCTOPLUS!!

After flash firmware root and follow the 16gb fix with custom recovery and comands
