octopus unlock h815 not success

i tried to unlock lg h815 g4 using octopus box but it doesnt unlock
it says unlock ok but when insert sim still says network locked...

i tried resetting the phone write back qcn which was backed up..but the phone still says network locked..plz octopus team what is this probem. i am using the latest version 2.0.4
