Ask gsmserver to change card, seems like your phone doesn't read it. Check this card with another iPhone.

Weird incompatibilities with SIMs, like this, are why I'm recommending the R-SIM Sup now. I was sometimes even getting "Call Failed" on OUTGOING calls, and it doesn't happen with the SIM only in the slot.
