I have iPhone 5s on iOS 12.1.4.

Also R-SIM12 v9.75.

Can I unlock my phone on this iOS version with my R-SIM?

I have tried but every time I restart it takes me back to the activation screen.

I strongly recommend to use R-SIM 13 it work perfect editing the last iccid

do you recommend to use r-sim13 or that is a must for ios 12?

Newer iOS version need R-SIM 13 also it's a cleaner solution. Activate with R-SIM 13 and put your preferred SIM you only need R-SIM 13 again just in case you restore, even if you update iOS you won't need it again.
