Between your iphone info and mine info, in the end, mine says locked policy and yours says magic policy.
Please connect iphone with itools and in details see if you have iphone6.1 or iphone6.2. Mine before used iphone6.2 which is for CDMA but after restoring iphone with sim card inside i was able to restore with iphone6.1 firmware.
Please check that.
If you remove sim card, does it say in actiation screen, "activated on CDMA network?".
