When the Imei is in 35000000 that means that the EFS folder is corrupted and the security of the phone is damaged.
If either SW version is unknown, or HW Chip is unknown, or HW Modem is unknown - most likely phone's firmware firmware or
EFS is damaged, or you have written unsuitable firmware. Also, this can be HW problem.

You should re-flash the device with different firmware one by one and check, if modem is correct (not Unknown).
If after writing of all firmwares, which you have, problem remains the same - most likely,
this is HW problem (problem with eMMC flash chip).

when you are able to start an get info process it will be great, from the log we can check many information.
But I think gcceleghg has right and the device's EFS damaged.

Hi, just reflash device with combination firmware check status on recovery mode... perform full wipe and check status again... post pictures...
