First - HTC phones never ask for codes without the sim card.

So my options are:

1. Try again Legacy tab (now when your phone is S-OFF)
2. Try SD mode tab - you can try to lock your phone with known code and then use it to unlock.
3. Try to reflash your phone, we have over 10 firmwares, just search for PD98

Finally done by XTC clip by Rascal after many try almost all clips in market
but XTC done on one shot.
