This is because either you jumpers are not properly soldered or are too long causing weak or noisy singal line.

Or you have not connected VCC or VCCQ. You may plug USB cable to phone board if don't want to solder VCC and VCCQ.

If you have original UMT Emmc Hardware tool then connect with isp point and after connect report here
