Hi,
about permanent "formatting" message:
In most cases it's not a software problem but hardware problem (damaged flash IC, bad soldering).

Hi,
In your phone now 2.0.65 firmware.
Flash it f.e. with 1.0.03 and phone will be unlocked.

Hi,

Flash with 1.0.03 again and select different Flex files.
Power On phone, Power Off phone, make unlock again.
Phone will be unlocked for sure.

Please, no need to make and attach screenshots here.
It's much more faster copy/paste text from Log window.

Next suggestion:
power on phone, power off phone. Insert sim and see results.
If no luck: use other Flex file.
