Cannot unlock Samsung phones with Samsung Code Reader (Pack 12)

I am trying to unlock some Samsung Galaxy phone with Samsung Code Reader (Pack 12). The phones I am trying to unlock are Samsung SGH-I317, SM-G386T and SM-G900T. The problem when unlocking the phones is either "NO_PORT_FOUND" or "Out of memory". Here're the actions and results I have with each model:

Samsung i317 (Galaxy Note 2)
1. Settings > Applications > Developer Options > check USB DEBUG MODE
2. Dial *#9090# and select "DM over UART"
3. Dial *#0808# and select "DM+MODEM+ ADB"
4. Reboot

I suggest to retry using FG_Sam_v1.0.6.0_beta1.rar since thats the latest version

I tried the new version, still no luck. I also installed the Samsung Android USB Driver from this site: https://developer.samsung.com/android-usb-driver, still not working. Please advise

I tried again with a different phone model (Samsung SGH-i547), same problem. Here's the steps I have taken and the results. Note that *#9090# does not work on this model:

Samsung SGH-i547

1. Settings > System > Developer Options > checked USB debugging
2. Dialed *#9090# > nothing showed up, I skipped this step
3. Dialed *#0808# > selected "DM+Modem+ADB"
4. Restarted phone.

I must be missing something. Is there any driver I need to install? I already installed Samsung Android USB Driver as mention in the previous post.
