Try to full Format with CM2MT2.
In case there is problem try with CM2MTK and post result, use same DA- Lava : Z61

Make IDENTIFY with holding SHIFT KEY ( hold it before and while pressing IDENTIFY button ) with active BACKUP option.
Sent me in PM log of identify and BACKUP of this device ( zip whole MT6739_UID folder )
