I tried to format but always same error:

COM6 [VCP0] selected
module v3.62
Action: reset settings ("format ffs")
Model: LG-KG195
Checking data, wait...
Phone should be Switched OFF !
Press and HOLD Power button now !
9600 autodetect
error: boot phase #2/1 failed
Can not initialize test mode, possible reasons:
1. communication fault, repeat required operation
2. ffs/eeprom is seriously damaged, use repair ffs/eeprom files or use another unlock/format method
error: operation failed !
Finished, time used: 1 min 39 sec
Remove/Install battery now !

Even if i remove battery from the phone and connect it to the cable! same error. why???

Mate First your cable or Jack problem than do as it shown in Infinty Documentation Folder:
Go to > C:\Program Files\InfinityBox\Documentation\Unlock_and_repair\ LG

it's for sure a cable and/or pinout problem.
