yes i did connect it and its always disconnecting after a short time and even when i write or erase or anyting and this is the log thats why i asked for the voltage and bus speed

Check CLK Resistor...
if any doubt then Just remove CLK resistor & make a Jumper on CLK line...
After that just Power ON the SET

here CLK resistor not present, So no issue with CLK line...
So, check with Microscope & check CMD line area...
