We will prepare test Software version, which performs "Repair IMEI"/"Unlock" operations via ADB, not via Diagnostic port. Please wait, this will occupy some time.

As I see from this log, modem part is damaged in your phone or You have written unsuitable firmware:
Quote:
SW version: Unknown
At first, try to re-flash the phone with different firmwares and check whether modem is correct and only then perform "Repair IMEI" operation.

SW version: Unknown
Please re-flash the phone with another firmwares (try 4.1.2) and check, whether modem is correct. If the problem will be the same with all firmwares, which You will test - this can be HW problem.

Phone was working, just so damaged frame - you know - there SGN loose metal painting very fast and come so ugly ... I have another board from phone with damaged PCB, but want to fit it in numbers on back sticker, just to avoid stupid questions when sale it. Do not think that will be a problem. I was use Octopus / Octoplus Samsung tool due to count less issues after Samsung Android phones EFS repair regarding other tools at me ( Z3x, SPT, NsPro, SGTool ) when catch it on WaveTek for testing after this. Anyway - will open it again - hope not damaged by mistake when assembling.
