Please try this guide:

Power the phone ON
Open Qcom tab
Install required drivers for diagnostic port
Connect your phone to PC with USB cable
If diagnostic port is not detected, power the phone ON and activate it by dialing *#*#76937#*#* Then select diag port in the ports tab.
Go to IMEI tab, enter your IMEI and press "IMEI Repair" button
