Your phone has damaged boot loader. Unfortunately phones in such state can not be processed with Octoplus/Octopus LG Software.

You can try to restore the phone with Medusa Pro Software.

Please, this phone model D295 is supported, check list of supported phone model ,if You have another phone model then please, next time create new thread and describe your problem there.
