Based on your log can be one from this what cause this problem.

1.Your exe is very old and can cause problem.
How to fix: Update your exe to latest version.

2.Your firmware can be wrong or incompatible
How to fix: Find compatible firmware

3.Phone can have hardware problem
How to fix: Perform hardware repair.

Also can be bad cable, discharged battery e.t.c.
But this most posible you check already.
