Phone remain in flash mode after unsuccessful connect or handle remain opened.
This can be solved restarting phone(keep for 10..20 sec vol- and vol+ and power on) untill phone will restart.
If phone have detachable battery, need to reinsert battery.
Also restart exe.
Phone must be charged.
If phone is not charged also will give mostly same error.
