You only need to enter the first 5 digits,
First two digits are the factory which made the phone
Third digit is year (2011 in this case) (8 = 2008, 9 =2009, 0 = 2010, 2=2012)
Fourth & fifth digit is the week of year:

Example With your sn / 88225KSQEDG:
88 - Factory
2 - year 2012
25 - Week 25 of 2012

The result checker is showing:
Blackgroud Color RED and show Error Messages: Wrong Year Inout It Should Be Number
iPhone Made Week: = 25
iPhone Made Year: =

This means you can not do iPad baseband, you can not change.

Any phone made on the 28th week or after of 2011 can be destroyed by flashing the iPad baseband. WILL DAMAGE WIFI,IMEI,BASEBAND...
