Save IMEI settings to clip, connect clip using Y cable to POWERED OFF phone, power on phone into fastboot using VOL-DOWN + PWR and go to simlock.

All these steps are described in manual...

Friends! Read manual! Why would you wait for an answer, when most questions are already explained in the manual or on the forum! Save your time.

Answer: You need to use Y-cable with this.

I have to have the bootloader locked ??

Bootloader state doesn't matter.
Remember to connect clip BEFORE powering on phone.
Or better with clip connected via Y cable:
1. Goto fastboot -> reboot bootloader
2. Wait for reboot
3. Goto simlock and choose it.
4. Wait for the process to finish.

SD init failed?
Make sure to connect y cable before powering on, or do reboot-bootloader from the fastboot menu prior to launching simlock.
