You must connect clip before powering phone on. Otherwise you have to press 'reboot bootloader' prior to using direct mode.

1. Connect your clip to PC to make sure it's detected, read files using XTC2Tool - does it read files OK?
2. The good idea is:
a) power off phone
b) connect clip to phone
c) power on phone in bootloader mode
d) goto fastboot mode using phone menu
e) Use XTC2Tool in direct mode and use "Write security flag" and set it to 0.
f) Post result here.

Please explain your problem in details.
1. You cannot turn phone to S-ON just by "hooking the box"
2. Some commands and one some phones like writecid will turn S-ON, you simply need to do S-OFF after this.
3. Please post your model and the way you are trying to S-OFF, your logs only shows that you are reading the security flag.

And next time please be so kind to create a separate thread for your problem to help us to support you better.

You can flash it to original firmware and it will remove the tamper flag.
However, we will consider adding this feature to the nearest updates.
The flag itself doesn't affect the way your phone works, and if you are intended to take it to your service center, you should know that they will detect modified firmware even without the tamper flag and may refuse to service it.
So if you need to pass it to service center, it's always good to reflash with original firmware.

I asked you to post log, but you didn't. I cannot help if you provide us only with "tried all options it didn't work".

You may try to reset your clip using this procedure:

If you did everything right, and connected clip before power on, and got this message - SD/USBDISK Init error, then there is only one case - your flex is damaged.
And what do you mean by "same reader java card work" - are you saying that your clip is not detected on computer?
