1 - I do not recommend using 36 Mhz , that could be one reason .
2 - If keep failing at same adress , try changing adress if passes could be bad block in that area / block
3 - Disk Damage ( Hynix ) = can be solved deep erase most of the time
4 - Write Protect = but in your picture that is not write protect error
