Which problem have with your Box ? It is only in Bootloader Mode ?

Looks like your box already were updated, and shouldn't make any problems . Give us more clues about this problem .

Open The box (Ask your reseller about) and fit properly the smartcard in the reader.

99% card is not fitted well in smartcard slot, as Moulnisky said open it and reconnectr.

Yes is possible, also you may check MCU solderings between Card .
Or put any regular sim card (GSM one let say) and check if ATR will be readen.

Sorry to say but that means your card is FAULT and need be replaced

Or ask your reseller (or GPG) to send you another card.
Box part responsible for smartcard is veryvery ok.
