I'm keep getting this while trying to unbrick LG G4 LS991
Download Fail: FireHose Fail FireHose Fail: Failed to send Firehose NOP to the phone.

Problem History:
My LG G4 LS991 got into Qualcomm HS-USB QDLoader 9008 mode while i was trying to flash it. and whenever i connect it to pc, the mobile is being detected as Qualcomm HS-USB QDLoader 9008 by itself.

Is there any solution for this? i'd much appreciate your help
