Hi,
please check the instructions and information in a logs too.
1. On the dial window, enter ##3424#
2. Select QUALCOMM USB SETTINGS
3. Select DM+MODEM+ADB
4. Click the Ok on phone display
5. Wait a few seconds until the diag port is initialized
6. Click Ok button
