just make the rpl backup and certificate backup first, as i can see your IMEI is Ook then do the SelfTest for ST_SIM_LOCK and ST_SECURITY if anyone failed then do write PM without Field 308 and 120 using SX4 if still not solve then full erase your phone and write latest firmware then do restore RPL and certificates, and then again do write FULL PM using SX4, hopefully this will solve your problem.
