One question, did you use your original flash backup or you used
another flash from somebody elses site? The symptoms that you mentioned are usual to phones that are uploaded with flash having different PPM package than that of its original flash(i.e. Flash A with C). Try uploading it with a flash that has the same package as your original.
Changing loaders should work on the problems as posted earlier. However, if symptoms persist, try uploading the whole flash 64kb at a time (i.e chop it into 64kb chunks).
