Before what was set condition it was locked/OK? or Dead
this problem comes when u r using pinfinder/battery r not charged/cable problem use power meter instead of battery
if problem is solved by this procedure check HW Resold/change Flash chip

Yes, I agree with Mr.maplegsm
if phone can not be flashed it is need to check phone hardware, f.e. replace or reball flash IC.
