If u have this problem , in universal box software go in flash tab and than do
set factory defaults
and than try to unlock it , will work 100%

I plus a DC power and try this again,it work!!!!

If u try all and it still doesnt work, here is what I did
I put phone in Local mode, then click UNLOCK and phone unlocked 1 time

actually this problem is because of low usb power, so we prefer external power.
