same problem ,
i have a t479b from virgin mobile canada,
i tried samsung mode and qualcomm mode

Using Qualcomm HS-USB-Diagnostics *#782872#
Reading Phone Info... OK
SW Version: T479UGJH3
Searching SysInfo... Not Found
Done with Samsung Tool ver 11.6

Using samsung mode *#726872#
i selected samsung modem and i get this when i click unlock
