Hi
Goto "DCC Read/Write" Page
Press "Setting By Code" Then Select "Read xx"
Unselect "Auto Full Flash Size"
Now,Select 128 or 256mb in "Data Length" List
Press "Read Memory"
Then after done,Press "Save" and select "Name for File" and Target for Create file

Dump looks okay, so this could be some other problem not related to software.
Do You know phone history, and have You checked if battery is fully charged?

If phone is not detected on usb port as "QHUSB" it means that You just have to charge battery.
"Dead" phone would always enumerate in QDLoader mode (QHUSB).
