Try without PS Hold and post the result here

PS Hold is a test point that designed to keep CPU powered Up while supplying Battery power

(It's average is 3.6 to 4.2v )

we should use something between 1K and 4.7K

So need to connect resistor (1k to 4.7k) between PS_HOLD TO Vref ?
