Try to perform "Read Codes" operation instead of an "Unlock". Post here result and full Software log of performed operations.

Try to do the next:
1. Make "DRK fix".
2. Reflash the phone.
3. Root the phone.
4. Try to perform "Read Codes"/"Unlock" operation again.

Post here result and full Software log of your actions.

Unfortunately the customer has gone with the phone. Thanks 4 being there.

Please try to perform "Fast Unlock" operation on different available COM ports, with enabled "Manual select" option.
If problem will remain the same - re-install Samsung USB driver, restart PC, then try to perform "Fast Unlock" operation again.

Such problem can be caused by new firmware, which has your phone. To restore the phone to its initial condition You need to write Security backup, created during "Unlock" operation, back into the phone.
To process the phone try to write older (by release date) firmware, then repeat "Unlock" operation.
