Seems like you have a phone with firmware version that requires patching.
Please provide us with such information:

What network provider / country current phone is locked to?
What is signed at the phone's screen when you power's it on with not original SIM card?
Please upload Scurity backup read from this phone to any filesharing web site and post link for it in a current thread.
