LG H791 screen or bootloader lock help

When power up the phone after google logo loading its ask for pin I am not sure that a bootloader lock that I already do factory reset.

I do bootloader unlock but I can't what model should i put the phone on it?

Its display that phone has been programme with different operations

the phone restart the freeze on "WARNING.. Not Support QEM = 0"

how can I flash with .oct flash file?
