There is can be few reasons :
1. Downgrade ( almost possible ) you post not first try log, seems, so can't say exactly what has been inside phone before.
2. Wrong RM.

If you flash other sw version ppm/cnt/jmpt - then it can go to such state. Try find first flashing log from it. Preferably when it was fully alive. Is it was?
