I am not sure what you are doing but I have done 13 units now without any issues. Here is a link to a youtube video proving LTE works after IMEI change:

Okay this is how you do it:

1: Change or Repair IMEI with Octopus (complete entire process)
2: Flash NEXUS4 from file with QPST. (SPC CODE 000000)
3: It will give you error message that it cannot reboot phone. Reboot phone manually.
4: Flash NEXUS5 from file with QPST. (SPC CODE 000000)
5: Reboot manually again.
6: Write original or new IMEI using EFS pro. (SPC CODE 308508 to connect to phone)
7: Reboot phone
8: Set mode to LTE automatic and make sure apn is checked.

ALL DONE!!!

Here is a link to my server with the files needed.

Alright let me get this right, I have to restore the Nexus 4 QCN to the Nexus 5 then the Nexus 5 QCN file and write IMEI using EFS Pro

I don't know why u need to restore QCN file for Nexus 4 on Nexus 5. Should not flashing just the NEXUS 5 QCN be fine?

NO just restoring the nexus 5 qcn will not work. Been trying that for months. So I started thinking why couldn't I use the nexus 4 qcn since there was no problems with the nexus 4 network settings after IMEI repair. So i tried that and after the nexus 4 qcn was written to phone i could only select 3g (not gsm or lte automatic) but got 3g instead of 2g. Then I thought what if I wrote the nexus 5 qcn on top of it (Maybe there is a setting in it that isnt in the nexus 4 settings that is in the nexus 5 settings and vice versa). And guess what? It worked. Now the Nexus 5 qcn file in the folder I uploaded might work by itself now because that is a qcn backup after I wrote both the nexus 4 and 5 qcn files to the phone. But if you just do a back up of a nexus 5 qcn and try to simply rewrite that back to the phone after IMEI repair you will only get 2g.

It is very strange, I am sorry to take your time and trying to figure this out because I use QPST a lot specially with CDMA phones to insert PRL file or even creating data profiles. However to my understanding when you flash a new QCN file it reset all the NV values to new QCN. Also flashing different QCN sometime make the phone dead or restarting/shutting off. Making a backup from original phone before flashing then edit the QCN with EFS Pro and flashing that backup should work but you said you have tried. Well if you got this working then you got the good find bro. Cheers and keep up the good work sharing your solution. :)

Here's what I have done.
Like many others, my Nexus 5 would not get 4G after IMEI repaired. So, I followed hellothere777's instruction.
At first, not sure how to get to DIAGNOSTIC MODE, but I notice you can get to DIAGNOSTIC MODE from LG Tool (octopus). So I tried to repair IMEI again using LG tool, then when I see "change to diagnostic mode", close LG Tool, then open up QPST, the software will see the phone.
From there you can proceed to load the Nexus 4 QNC file, reboot, repeat the same procedude for Nexus 5 QCN, reboot then diagnostic mode again, then change your IMEI with EFS pro.

It worked for me :) get LTE with Tmobile and AT&T.
